I'm not OP, but I've read the Manning/Lamo chat logs, and it sounds like reality matches pretty closely. According to Manning, most of the world's communications are routed through NSA networks. The volume is so vast, however, they can't possibly sort through it all. According to Manning, they basically check for certain keywords or patterns to pop up and then decide what to do from there.

Also, NSA has some pretty serious encryption cracking hardware. Again, according to the chat logs, there was only a small handful of encryption schemes they couldn't touch yet. Can't remember the specifics now, but it all came down to resource allocation. Some schemes took a few hours to break, some took months, some wouldn't be touched because cracking would just take too long.

That sounds pretty close to the fiction of the big 3, Hollywood action movies notwithstanding.
